Prescription Refill & Renewal Policy
The best time to request a refill is at an office visit. At the beginning of each visit the nurse will ask you if you need any refills of medication. We prefer you bring all bottles of medication with you to each visit, so we may ensure you will not run out before your next routinely scheduled appointment. Planning ahead saves you time.
When not occurring at the time of an office visit, we request you allow 2 business days (48 hours) for us to refill your prescription. This time may be shortened by providing us all the information we need, as described below.
Prescription refills may be obtained in three ways:
1.) On line. This is fastest. Please obtain a patient user name and password and sign our permission form at the time of an office visit. Please have the information requested below (Required Prescription Refill Information) available when calling. Go to (www.CenterForPreventiveMedicine.com). On the home page, go to Patient Log in. Enter and follow prompts.
2.) By phone. You may call our prescription refill line 618-1100, prompt #3. Please leave a message with all the information requested below. We need all the information requested below to refill your prescription.
3.) In person. You may walk up to our waiting room window 8:30 to 11:00 and 1: 00PM to 4:00 PM M-F to drop off a request for a same day non-emergent refill. Be prepared to wait as scheduled patients are given priority and the physician will not be interrupted to write refills for non-emergent prescriptions until near the end of morning or afternoon sessions (11:00AM and 4:00PM Monday, Wednesday, Thursday, and Friday. On Tuesday only 1:00PM). Non-emergent refills will NOT be accepted between 11:30 and 1:00 PM as the front desk is closed at this time. Emergent medically necessary refills will be honored in person ASAP on a case by case basis.
Required Prescription Refill Information:
1.) Your name: first name, middle initial, last name. If you are leaving a message for the nurse by phone, please spell each.
2.) Your date of birth, work telephone number, home telephone number or cell phone number and e-mail address.
3.) The name of the medication, dosage (milligrams), and how many pills you take and how often during the day you take them. This information may be read off the medication bottle. If calling please spell the names of medications. Also, please include the number of month’s supply of pills you request the pharmacist place in the bottle you will pick up.
With the exception of controlled substances, we routinely will fill a three month supply (90 days), with 4 refill remaining-a total of over one year of medication before your next refill.
4.) The name, address, and telephone number of the pharmacy to which you would like the prescription sent. If calling please spell the name of the pharmacy, location and phone number.
Additional Required Prescription Refill Information for Pre-Authorization
1.) For prescriptions requiring pre-authorization, we will need you to fax us
the face of your current health insurance card, Medicare card and/or

Please note: Emergency-same day-refills requested which are not a medical emergency and without an appropriate explanation may be billed a $10 processing fee.
Refills of narcotics or controlled substances will, in most instances, require an office visit every other filling. Some narcotics allow for refills and others are restricted to a 30 day supply. Also, some of these medications cannot be phoned into the pharmacy and must be picked up at the front desk.
